I've seen this lots of times in my career. Almost always on old rovers with the Buick v8. If you're lucky, just a gasket failure allowing coolant to mix with oil. If you're not so lucky, cracked block or head. Sometimes it will cause the engine to hydrolock and shatter pistons or bend rods, or just kill bearings and cams.
